Alonso Sanchez and Miguel Ochoa were performing fireproofing work on reactor supports at a customer’s job site. While performing their work they discovered a leak that was under some insulation. They reported the leak to their Brock supervisor and the customer’s operator who then wrote a work order to strip the section of insulation so that the leak could be repaired properly.
